I’m not sure it’s ever entirely possible to end confusion when multiple characters have used the same costumed identity.

Right this moment, Scott Lang is Ant-Man. But it wouldn’t be inappropriate for Hank Pym to break out his Ant-Man gear if the circumstances called for it. And Eric O’Grady is still out there as well, these days as Black Ant, but potentially another guy who could turn up wearing the Ant-man’s colors.

So no, somebody talking about Ant-Man in a story doesn’t necessarily mean that they’re referring to Scott. It all depends upon the context of what is being said and who the speaker is.

Which is a lot more complicated to deal with, but there you go.
